B) Going up country. Once located in a particular country the nurses felt a responsibility to provide services for the whole community. This was often difficult, as expressed by this nurse:

We travelled several days to reach the village, but we could only really help half the people. This is because the village is separated by a border and we couldn’t cross it and that is tough.

With many countries at war there were dangers if they strayed out of a designated area. There was general acceptance that it was reasonable to expect to stay put in dangerous circumstances but that more often the regulations seemed petty and limiting. It was felt that these regulations were put in place without either local officials or agencies understanding the main purpose of health clinics and what nurses do. For example, nurses were often perceived by officials as mainly distributing medicines and treatments in clinics and so had no need to go outside of their designated area: The rules also seemed to apply to different people in different ways. One nurse pointed out how a woman doctor, who was working in the same village, was not expected to comply with the same restrictions. Unless specific instructions were given NOT to do something it was generally agreed that it was better not to ask but to go ahead on the assumption that it was acceptable.
