______ Has there been any history of depression (diagnosed or undiagnosed), adjustment disorder, anxiety, or any other mental health diagnosis?

______ Are there any other medical problems occurring (e.g., hypothyroidism, hyperthyroidism, Huntington’s disease) that may mimic other mental health disorders?

______ Has the client previously or recently been admitted to any type of skilled nursing facility for any reason (physical health or mental health)?

______ Is the client lacking a strong social support system (children, family, friends, life partner, spouse)?

______ Has there been any significant recent loss of a loved one?

______ Is the client withdrawn from their facility, home or treatment community (participating in social activities)?

______ Is the client currently being treated for any medical conditions? If so, how are these symptoms being managed (prescription medication, alternative medicines, over-the-counter medications)?

______ Is there any history of prior suicide attempts or NSSI behavior?

______ Is there any history of drug and/or alcohol abuse? Is there any current drug and/or alcohol use?

______ Are there any current significant life events (loss of income, loss of home, identity concerns)?

______ Does the client have access to medical care (insurance, service providers, mobile service providers)?
