Key stakeholders in ‘contracting-out’ arrangements are the ministry of health, district-level health authorities and the health sector reform cell (if involved in such an initiative). The success or failure of this model is often determined by the local community, public health staff, the private agency and the facility level advisory committee. The community is an important stakeholder because its members might perceive such a partnership as an attempt at privatization and could mobilize the community against such an arrangement—as experienced by Karuna Trust in Karnataka. With entrenched interests in the local community, public sector staff might connive with either the unions or local interests to sabotage such a partnership. As outreach staff, community health workers may not like to report to a more demanding new management. In this dynamic, patients are important beneficiaries since an affordable primary care service is available at accessible distance run by more ‘courteous’ and ‘considerate’ staff.
