They had a consillium today. That’s the weekly meeting held by the Ministry of Health doctors and the two MSF expat doctors to discuss various patient situations. These may be ones which require a change in drug regimen due to onerous side effects, or that someone has missed so much treatment that they will be listed as a defaulter, or someone has not responded to treatment and needs to be declared a treatment failure and taken off the drugs. Sometimes community TB doctors attend if it’s their patients at issue and sometimes a counsellor will attend. I sent Deelya to this one because they were to decide on Oleg’s situation. I had talked to the TB1 doctor and Firoza our MSF doctor and told them Oleg wanted to go to TB2 and accepted that he now had to fight full MDR and was ready to take the larger drug regimen.

I was an idiot.

At the consillium, Deelya was overpowered. The chief doctor at TB2 wanted to know if Calvin could guarantee that Oleg would take his drugs every day. Deelya replied that there are no guarantees any patient will do that, but Calvin thinks Oleg will do it. What Calvin thinks is not good enough. Oleg will be sent to TB3 as a treatment failure. They wiped their hands of the problem. No more drugs.

No more hope.

When Deelya tells me the outcome, I am devastated. I want to tear the walls down. They asked me to help him decide, but now I see it was a lie, there was no deciding. No one wants Oleg. I frantically phone Samuel. “Yeah, it was too bad. You should have been there. Deelya tried but they would not listen to her. I think if you had been there then the decision on Oleg would have been different.”

I want not to be in my skin. I want to rip it off and disappear. This is too much for me. Oleg has no one. Not even me.
