The scale or notes used for each mode will be right under the mode name, which you can see here circled in green. Some other books show only a part of the scale, but here every note on the scale is shown. After the 12th fret, the notes just repeat again. For example, the 13th fret would match the 1st fret, the 14th would match the 2nd etc.

The red, circled note is the root note of the scale. All that means is that you should use this note frequently to amplify the sound of the mode. Try to start and end your guitar parts (or vocal parts) with this note. You don't have to, but it helps bring out the distinct sound of each mode.
