Two days after surgery, Mr Carstairs reported feeling very ill, with nausea, bloating and sweats. It was a bank holiday weekend. The hospital was short-staffed. A series of junior doctors reviewed Mr Carstairs but reported nothing untoward. The senior doctor that weekend was a name I did not recognise. He signed himself "Locum registrar". On the Tuesday following the bank holiday, an entry in the notes indicated suspected leak from the anastomosis, confirmed on X-ray.

Mr Carstairs went back to theatre where the anastomosis was found to be leaking. It was repaired, but by that time sepsis had set in.

He died five days later of multiple organ failure, as a result of infection stemming from the surgery he'd undergone to treat his secondary bowel obstruction from adhesions.
