health. The self-punitive loop illustrates that cognitive distortions relating to the skin can result in clients engaging in internal cognitive attacks that constitute an internal shaming process. The use of diagrammatic formulations empowers clients to choose at which point they wish to break the vicious circles. Again, due to the sensitivity of the subject, therapists should be aware that diagrams may be unintentionally shaming and should be aware of this possibility. Clients can be requested to state and rate their affective responses to the development of the diagrammatic formulation in order to be sensitive to shame exposure issues.

Acne suffers, like those with other facial unattractive traits, coming for psychological help will often have either social anxiety and/or depression as part of their shame (see Kent & Thompson, Chapter 5, this volume). If social anxiety dominates the clinical picture then the clinician may be guided by the current work on social anxiety that focuses on meta-cognitions and safety behaviours (e.g., Clark & Wells, 1995) and in some cases groups may be useful (Bates & Clark, 1998). If depression dominates the clinical picture then again therapies developed for depression (Beck et al., 1979; Blackburn & Twaddle, 1996; Gilbert, 2000c) may be useful. In some cases, people may have very self-critical and self-hating feelings towards themselves and their skin disorder. Recently, Gilbert (2000b) has suggested ways that the cognitive approach to working with such negative self-evaluations can be modified to help develop a more compassionate self-structure. To date these interventions, borrowed from work with depressed and anxious people and modified for work on acne, have not been subjected to clinical trials. However, there is good reason to believe that they will aid in the development of therapies for those who have serious psychological problems associated with their acne.
