Chapter 10

Michelle had, over the course of her career, built the cardiovascular ward at Silver Memorial from the ground up. The entire process had been plagued by problems, administrators who didn’t want to give up control or other medical staff not ready to let a young, hotshot female doctor order them around. But she’d made it work through perseverance, maintaining tight control on the ward and everything that happened inside it through her commitment to providing excellent, life-saving care for everyone who came to her practice.

Now she felt as though the world, and the ward itself, was crumbling down around her. The sounds of the hospital cafeteria felt distant as Michelle stared into her coffee, unable to bring herself to take a sip. Her mind replayed the events of the past hour over and over again in her mind. Tom Winters had been fine, he’d recovered from the surgery and his incision was healing well, and then he’d gone into acute septic shock. The ICU was doing the best they could to stabilize him, it was out of her hands.

Across from her, Lydia was huddled on the bench with the same shocked expression Michelle could feel on her own face.

“Dr. Foley, I need to tell you something,” she said, her voice quiet enough that Michelle had to strain to hear her. It was the voice of someone crushed with guilt. Michelle wondered what Lydia could possibly be about to tell her that would justify it.

“Go on.”

“When we were doing our evening rounds, I noticed a discarded syringe in Mr. Winters’ trash can.”

“In his trash can?”

“Yes, not in the medical waste bin like it should have been. I was going to bring it up then, but I forgot.”

Michelle rubbed her forehead. “I’ll see if the trash can has been emptied, but it’s probably nothing. The nurses are following aseptic technique for him, it’s likely it was the syringe used to reconstitute the cefuroxime, it would only have contained sterile water so strictly speaking it’s not medical waste. Sometimes people use the trash can closest to them instead of what they’re supposed to do.”

“I have a bad feeling about this, Dr. Foley.”

“I have a bad feeling about all of this. I have to report to the administration what’s been going on. We’ll see what they decide to do about it. After Infection Control came and talked to everyone, I had hoped Mr. Song was the end of it. But with Mr. Winters, I’m concerned that someone is not following protocols—the combination of aseptic technique and the fact he was on post op antibiotic prophylaxis means he should have been the last person to develop sepsis.”

“Do you think that it could be just a coincidence?”

Michelle shook her head. “I don’t believe in coincidences. I believe in patterns. And two isn’t enough to make a pattern, but I’m not about to let this keep happening if I can stop it.”

Lydia nodded, hands clasped around her coffee. The irony of the situation wasn’t lost on Michelle as they sat there, both of them quiet as they tried to process what was happening.
