Summary

Stapled hemorrhoidopexy is recommended as a treatment option for symptomatic prolapsing piles. It may be useful in grade II prolapse which have failed to respond to nonexcisional therapies. The evidence would suggest it is of maximal benefit in grade III and IV hemorrhoidal prolapse, although the recurrent prolapse rate appears to be higher if the volume of hemorrhoidal disease is large. The absolute and relative contraindications to stapled hemorrhoidopexy are outlined.

The STARR procedure is indicated in patients with ODS associated with redundancy of the distal rectum as manifest by rectocele and/or internal rectal prolapse. When considering patients for STARR it is important to consider ODS as a complex, multifactorial condition which is commonly associated with other pelvic floor pathologies. A combined multidisciplinary approach is often necessary. A treatment algorithm for STARR is provided.
