Question 10

In head and neck malignancy, perineural spread of tumor to the cavernous sinus usually occurs through the following CNs: maxillary (V2) and mandibular (V3) divisions of the trigeminal nerve (CN V).

Top Tips

Dural “tail” and narrowing of the internal carotid artery are highly suggestive features of cavernous sinus meningioma.

Lymphoma and pituitary adenomas involving the cavernous sinus do not narrow the internal carotid artery.

Inflammatory pseudotumor of the cavernous sinus (Tolosa-Hunt) can mimic cavernous sinus meningioma on imaging, but the clinical presentation is different.
