78b Answer: A.The normal diameter of the main pulmonary artery is approximately 25 mm. The 90th percentile cutoff value is 29 mm in men and 27 mm in women. A measurement of 37 mm, as found in this case, is highly likely to be associated with pulmonary arterial hypertension. A recent meta-analysis showed that pulmonary arterial hypertension was found in about 13% of subjects with scleroderma, compared with 3% of patients with lupus. The prevalence of pulmonary hypertension in subjects with Sögren syndrome and polymyositis–dermatomyositis is low. In patients with scleroderma, the NSIP pattern of lung fibrosis is found in 78% of surgical lung biopsies, substantially more common than other pathologies including UIP.
