CONSIDERING MUSCULOSKELETAL GROWTH AS A TRAJECTORY

We have discussed complex systems, organisation of systems, trajectories in state space, system criticality, epigenetic landscapes, the adaptive possible, and entropy. How is all this relevant to the clinical management of musculoskeletal deformity in children with cerebral palsy? Waddington’s epigenetic landscape and the concept of state space and trajectories of growth and development could be used as a metaphor for system development as well as cell development. This concept would apply to the skeleton as well as to skeletal muscle; we could, for example, consider the development of the hip joint in a child with cerebral palsy in this manner. Rather than hip development resulting in either typical development or hip dislocation, there is a range of potential outcomes with a normal hip at one extreme and a dislocated hip at the other. These could be viewed as possible pathways within a branching diagram, analogous to the valleys suggested by Waddington in his epigenetic landscape.
