Vaccination recommendations

Hep B vaccine is injected deep into the arm muscle, or the thigh of infants. The recommended doses are as follows:

Four 0.5ml doses for all children, at birth and at 2, 4 and 6 (or 12) months. The last three are given in a multivalent (combined) vaccine that includes diphtheria, tetanus, whooping cough (pertussis) and sometimes polio and Hib. A Hib-hepB combination is also available. If the birth one is missed, the three others are sufficient. If the mother is a carrier, the birth dose must not be missed and must be given as soon as possible after the birth, and hepB immunoglobulin should be given as well.

For children and young adults under twenty years who missed the infant vaccinations, three 0.5ml doses are recommended, with 1 month between the first two and 5 months between the second and third. (Adolescents can also use a two-dose vaccination of adult H-B-Vax II 10ug.)

There is not universal vaccination of older adolescents and adults in Australia, but those at risk should be vaccinated. Three 1ml doses are given, with 1 to 2 months between the first two, and 2 to 5 months between the second and third. Those at risk include: – household contacts of people with hep B

– people with multiple sex partners, especially male to male

– injecting drug users

– people on dialysis and anyone with an impaired immune system

– people who receive blood products for clotting disorders

– people with chronic liver disease or hep C

– residents and staff of homes for the intellectually disabled

– inmates and staff of jails and detention centres

– health care workers, emergency services personnel and embalmers

– long-term travellers to countries with a high hep B incidence

– liver transplant recipients.
