Conclusions

Social anxiety disorder is a common comorbid condition in people with schizophrenia. It is often missed in clinical practice, with clinicians tending to assume that lack of socialization is due to the negative symptoms of schizophrenia or that the individual is responding to positive psychotic symptoms. Specific questioning regarding the core cognitions associated with social anxiety disorder (i.e. fear of negative evaluation by others) is required to delineate the disorder. Treatment with cognitive behavioural techniques (preferably group-based) can be effective in ameliorating the social anxiety symptoms and improving social integration and quality of life. Certain antidepressant drugs can also be effective, but side effects, drug–drug interactions, and potential for worsening of psychotic symptoms need to be weighed before adding them to antipsychotic medication regimes.
