Increase speed of onset

Using the principles of motor learning, it would seem reasonable to suggest that, to increase the speed of onset of muscle contraction, treatment needs to use normal functional activities that will produce a need for the muscle to contract. With delayed activation of transversus abdominis in patients with low-back pain (Hodges & Richardson 1996), treatment would include functional postures and movements that challenge postural stability and this would need to be continued repetitively over a period of time. The use of a gym ball and sit fit (Sissel UK, Halifax, UK) may help to facilitate speed of onset of muscle contraction (Richardson et al. 1999). In patients with anterior knee pain, a combination of specific muscle exercise with biofeedback, muscle stretches, taping and patellofemoral accessory movements has been shown, over a 6-week period, to increase the timing of muscle onset of vastus medialis, when going up and down stairs (Gilleard et al. 1998; Cowan et al. 2002). The mechanism by which the tape increased the onset of vastus medialis is unclear. Cutaneous stimulation has been shown to alter recruitment threshold and recruitment order of motor units (Garnett & Stephens 1981; Jenner & Stephens 1982).
