Lifestyle Modifications for African Americans with High Uric Acid and/or Gout
Lose weight
Avoid fructose-containing sugary-sweetened beverages
Limit red meat (beef, pork, and lamb) and seafood
Limit certain vegetables including potatoes, spinach, asparagus, peas, cauliflower, and mushrooms
Avoid alcohol (particularly beer and liquor)
Increase dairy intake (eggs, low fat milk, yogurt cheese) if not lactose intolerant
Get regular exercise
Overall the treatment of gout in African Americans is no different than any other racial group or ethnicity. Patients should be counseled regarding dietary and behavioral modifications first. Suggestions should be specific. African Americans should avoid fructose-containing sugary-sweetened beverages (“high fructose corn syrup”) [54]. Red meat (beef, pork, and lamb) and seafood should be significantly limited. Certain vegetables also lead to higher uric acid levels including potatoes, mushrooms, spinach, asparagus, cauliflower, and peas [54, 62]. Remember to assess the patient’s alcohol intake, recommend a decrease, and particularly discourage beer [62]. Daily intake of coffee has been shown to be protective of gout [63], as has vitamin C and cherry juice [64]. Although there is increased lactose intolerance in African Americans, those not intolerant should increase their dairy intake in the form of skim milk, cheese, and eggs [62, 66]. A study by Dalbeth and colleagues found a decrease in uric acid levels of 10% in those that consumed milk [65]. Like in many disorders related to lifestyle habits, exercise and weight loss are always beneficial [66].
