iIn this section I differentiate the physicians by their role at the clinic. The pediatricians (pediatric residents) are the actual expediters of the case and are responsible for the final disposition. The psychiatrists (psychiatric residents) function more as consultants on the cases.
jThe exam is actually constructed so that it can be numerically scored, but I have never seen this done. Perhaps this option for quantification gives it an aura of "objectivity." Most reports are made in terms of "He did well (or poorly) on the developmental, strong (or weak) in memory and sequencing, weak (or ...) on fine motor tasks. His style was somewhat impulsive, etc...."
kThis interpretation might be considered as constructing secondary meaning. That is, by creating (selecting) evidence the physician is creating primary meaning; when he interprets this evidence he is giving it secondary meaning. I refer to this latter process, for the sake of clarity, as interpretation.
lit is interesting to note that if there is a "late" onset of these behaviors then it is seen as evidence for a diagnosis other than hyperactivity.
mThe usual explanation for giving a physical during the evaluation is that other problems may be there to be detected. It also serves to reinforce the medical definition of the situation.
nWhen a child is over 11 years, however, it is not so clear. It is an accepted notion that in these older children the "signs" are often fading, and that the exam is not sensitive to pick up these "subtleties" in older children. The findings of the exam are thus more ambiguous.
