His pulse was weak to palpation, blood pressure difficult to measure but appeared to be low, approximately 80/45, tachypnea to ~25/min noted, and his extremities were cold and clammy.

His echocardiogram showed dilated left and right ventricles with biventricular systolic dysfunction; LV ejection fraction was 15%.

Tissue Doppler tracing from the lateral mitral annulus and pulse Doppler of the LVOT were acquired (Fig. 9.1).

Pulse Doppler at the LVOT is shown (Fig. 9.2).Ventricular trigeminy is seen on the ECG tracing on the images; every third beat is a ventricular premature contraction.
