What Clinicians Want

A recently published survey of pediatric residents in a single program identified better skills in pain management as the primary education need expressed by residents caring for children at end-of-life. Their comments implicitly recognize that pain management does not happen in isolation, and communication skills were listed as the next four most important education needs, namely to be able to communicate regarding prognosis, bad news, code status, and to be able to talk with a child about dying (Kolarik et al. 2006).

Nurses often report conflicts in ethical practice, in providing adequate pain management versus hastening death, and in truth telling to children whose parents are not willing or able to disclose the likelihood of their child’s impending death (Solomon et al. 2005).

A disparity between family and provider’ perceptions regarding adequacy of pain management at end-of-life was noted by Contro et al. (2004). A majority of families reported feeling that all that could be done to manage their child’s pain had been done, whereas the majority of staff revealed concerns that their pain management skills were inadequate to the task of caring for a dying child. Clinicians from different disciplines report the need for more personal support during the difficult task of providing compassionate end-of-life care to children.
