Cantilevers in partially edentulous patients have been widely used in the maxilla and mandible, primarily to overcome anatomic restrictions.44–46 Based on short‐term and medium‐term studies on cantilevered FDP, it is clear that there are no differences in marginal bone levels between partial FDPs, with or without cantilevers. However, mechanical complications are frequently observed in prostheses with cantilevers.46 This may be sufficient to warrant careful consideration and not to opt for cantilever restorations as a first choice in treatment planning. A systematic review by Aglietta et al. identified multiple mechanical complications in cantilever restorations and lack of evidence on the influence of mesial or distal cantilevers on mechanical complications.47 However, as forces are much higher in the posterior regions of the jaw, it can be suggested that mesial cantilevers may be preferable to distal cantilevers in partial FDPs. Within the scope of dental occlusion, clinicians should be prepared to deal with future technical complications when cantilevers are chosen. The difference in cost and long‐term patient satisfaction between use of a cantilevered prosthesis versus pre‐prosthetic surgery and placement of an additional implant, versus avoidance of cantilever with a shortened dental arch is an area that needs further exploration.
