15.3 Investigations

A preliminary working diagnosis is required in systemic disorders causing pruritus. Tests usually include a complete blood count and differential, biochemical tests for thyroid, hepatic and renal function, and a chest X-ray examination. Stools should be examined for ova, cysts, and parasites. Urine should be tested for albumin and blood sugar.

Patients with PUO should be considered to have an internal disorder unless proved otherwise. Additional tests such as stool for occult blood, Papanicolaou smear, and further radiological examinations may be necessary. Radio contrast studies and CT scans are usually not indicated as a part of evaluation in every case, but in some cases, it may lead to the diagnosis of a treatable disorder. Electrophoresis of the blood and urine is required in some cases of pruritus.
