Diagnosis

Diagnosis of hypercholesterolemia should be carried out by the primary physician. According to the National Institute of Health (NIH) and National Cholesterol Education Program (NCEP) guidelines entitled, Detection, Evaluation, and Treatment of High Blood Cholesterol in Adults (Adult Treatment Panel III), screening should involve 1) a lipoprotein profile obtained after a 9-12 hour fast, and the identification of clinical atherosclerotic disease that confers high risk for CHD events (CHD risk equivalents) such as clinical CHD, symptomatic carotid artery disease, peripheral arterial disease, abdominal aortic aneurysm, and diabetes (NIH-NCEP, 2004). From there, the identification of major risk factors other than elevated plasma LDL-cholesterol levels will allow for CHD risk assessment. Short term CHD risk may be assessed, and treatment goals established with the identification of risk factors, and the presence of CHD or CHD risk equivalents other than elevated LDL-cholesterol. NCEP recommends that diet and lifestyle modifications be initiated if LDL-cholesterol levels are above target, and pharmacotherapy initiated if necessary. NIH-NCEP screening parameters for hypercholesterolemia are shown in Table 5.1, and major CHD risk factors are shown in Box 5.1.
