Second Generation

As researchers learned more about the pill they realized that such high doses of estrogen and progesterone were not necessary. The earlier versions of the pill, called monophasic pills, also kept the dose of estrogen and progesterone fixed during the first twenty-one days, followed by seven days off. Second-generation OCPs involved lowering the dosage of these hormones, and varying the dose at different phases of the cycle: for instance seven days of 0.5-mg progesterone, seven days of 0.75-mg, and seven days of 1-mg. Pills with two doses of progesterone during the cycle are called biphasic; those with three different doses are triphasic.

Second-generation OCPs include norgestrel/ethinyl estradiol (Low Ogestrel), norethindrone/estradiol/mestranol (Ortho Novum), and levonorgestrel /estradiol (Alesse, Triphasil, Trivora). Later-generation OCPs typically have lower concentrations of hormones, which result in fewer side effects like hypercholesterolemia, blood clotting, and mood changes. First-generation OCPs and second-generation OCPs with levonorgestrel have double the risk of heart attacks as third-generation OCPs, which I discuss next.
