16.3 Amputations and Limb Reduction Defects

DESCRIPTION AND CLINICAL FEATURES

A terminal reduction defect is an abnormality of the extremity where the distal part of the extremity, including part or all of the hand or foot, is absent. Terminal reduction defects and defects with absence of an entire extremity most often result from an insult to the extremity in utero, such as an amniotic band or vascular accident. These malformations are not usually associated with other anomalies, and the prognosis is generally good.

Ectrodactyly, also known as split hand or cleft hand when only the hand is involved, is an anomaly of the hand or foot characterized by absence of one or more middle digits, leaving a gap between the thumb and remaining digits. The condition, particularly when both the hands and feet are affected, may be the result of a genetic mutation. When isolated, the deformity likely results from an amniotic band or vascular insult.

Rarely, reduction defects or absence of an entire extremity may occur as part of a syndrome. In such cases, the prognosis is related to the severity of associated anomalies.
