Personality Disorders

A 35-year-old man presented for an evaluation with a guardian from his group home. He was reportedly diagnosed with an intellectual disability as well as Antisocial and Schizoid Personality Disorders . He was considered dangerous and was not allowed to be without supervision. This man was prescribed a number of medications and given little freedom to leave his group home. In testing, he was quiet, polite, and expressed an interest in mystery novels and going for walks, which he was no longer allowed to do. He presented with repetitive and stereotyped language, interests, and behavior as well as deficits in social communication dating back to preschool. He had never been diagnosed with an Autism Spectrum Disorder and had never received treatment other than being educated in a special education setting. Because he had not been properly diagnosed, and treated, this man did not have the independence he could have possessed. He might otherwise have been able to hold a job or live at least semi-independently.

A personality disorder is an enduring pattern of inner experience and behavior that deviates markedly from the expectation of an individual’s culture. It includes cognition, affectivity, interpersonal functioning, and impulse control . The pattern is inflexible and pervasive and leads to clinically significant distress or impairment in social occupational or other important areas of functioning. The pattern must be stable and can be traced to adolescence or early adulthood and it cannot be better explained as a manifestation of another mental disorder (APA, 2013, pp. 646–647). Personality Disorders are clustered thusly: A—odd and eccentric, B—dramatic, emotional, or erratic and C—anxious or fearful . At times, an individual may be diagnosed with a Personality Disorder without a thorough developmental assessment and another mental or neurodevelopmental diagnosis may be overlooked.
