FIGURE 29.1. An implantable loop recorder (smaller device) and activator (larger device) used to monitor and record heart rhythms. The implantable loop recorder is implanted under the skin and can automatically record heart rhythm events. The activator can be used to trigger a heart rhythm recording when the person is experiencing symptoms such as palpitations, light-headedness, and dizziness.

An implantable cardiac monitor contains no wires (leads), can be implanted below the skin within minutes, and can remain implanted for up to three years. The purpose of the device is to monitor and record arrhythmias.

Information can be downloaded from the device, which can demonstrate significant rapid rhythms and slow rhythms that were previously undiagnosed despite a thorough work-up by a doctor. Many people who have received an implantable cardiac monitor have been discovered to have occult bradycardia, and these people benefited from a pacemaker implant. The risks of implanting a subcutaneous implantable cardiac monitor are mainly bleeding and infection. (See also chapters 30, 31, and 32.)
