After the patient square and the overlapping circles within it are drawn and discussed, another larger square can be drawn around the first square to represent the environment in which the person lives and works or goes to school. In this square, important environmental supports and stressors are noted. This could include supports like “lots of friends at school,” “playing on a little league team with a great coach,” or “brother who helps a lot with homework.” In the same space, environmental stressors can be noted: “Grandma is very sick,” “picked on by two kids on the school bus,” “company is planning a lot of layoffs,” “older sister gets drunk a lot,” etc.

Once this drawing is completed, the clinician can elicit reactions from the patient and family so they can express agreement or disagreement with the formulation, and can suggest important strengths or stressors that have been omitted. This can become a focus for recognizing differences of opinion in the family and can also provide a basis for discussion of what options are available for trying to improve the current situation of the patient and family.
