The signs of right ventricular hypertrophy are visible in V1 of Fig. 3.14, through an exclusive R wave of right ventricular depolarization with a voltage of 15 mm (1.5 mV), which is above the normal limit (see Table 1.3). Its morphology indicates right intraventricular electrical delay. In V6, the pattern of ventricular depolarization is normal so R/S > 1.

Since the T wave is positive in V2, the pattern of ventricular depolarization points to possible right ventricular systolic overload. The +110° right deviated QRS axis is normal for the age of this patient.

The patient suffered from a large perimembranous ventricular septal defect and pulmonary artery banding resulting from neonatal aortic decoarctation surgery.
