Core stability
âWorking the coreâ has become a huge focus of rehabilitation of athletes and nonathletes in recent years. The belief that the spineâs stabilizing muscles become inhibited with back pain, rendering the spine unstable and vulnerable, often drives this. It is worth noting that growing evidence tells us that persistent back pain disorders are often associated with increased trunk muscle co-contraction, earlier activation of the transverse abdominal wall and an inability to relax the stabilizing muscles of the spine such as lumbar multifidus (Dankaerts et al. 2009; Geisser et al. 2004; Gubler et al. 2010). So, while we will look at a couple of common âcore stabilityâ exercises, it is worth noting that focusing on periods of relaxation for these muscles is also important. Restorative yoga practices are a great option here. Recent studies have also demonstrated that positive outcomes associated with stabilization training are best predicted by reductions in catastrophizing1 rather than changes in muscle patterning (Mannion et al. 2012), highlighting that nonspecific factors such as therapeutic alliance and confidence in the physical therapist may be the active ingredient in the treatment, rather than the desired change in muscle.
