Screening for Osteoporosis

The US Preventive Services Task Force has not made a specific recommendation about resident screening for osteoporosis. However, they do recommend consideration of screening in women over age 65. There is currently inadequate evidence to recommend screening in men. On the other hand, the National Osteoporosis Foundation recommends screening in women over 65 and men over 70 [23]. This may be done with DEXA scanning of the spine and hip, quantitative ultrasound of the calcaneum or by FRAX risk calculation [24]. For many residents, they may have already sustained an osteoporotic fracture or have an elevated T-score. Optimal intervals for screening are undefined. Life expectancy, prognosis, and clinical judgment should determine which residents would be most appropriate for screening. For many, treatment with bisphosphonates or other agents may be tolerated and desired; for others, it is burdensome, and inconvenient and has potential adverse side effects.
