I hereby acknowledge that [REDACTED] is an experimental compound, which is currently undergoing classified DoD clinical trials for use in treatment of traumatic brain injuries (TBI). I authorize the administration of [REDACTED] solely under the supervision and care of DoD, USAF and/or US Army medical personnel specifically trained in treating cerebral and/or cranial injuries with approved and/or experimental therapies.
