How AROM Affects Monitoring

Sometimes AROM is necessary to place internal fetal heart and contraction monitors. External contraction monitors can only tell when contractions occur and how long they last. They don’t indicate strength of contractions. If a health care provider wants to determine how strong contractions are (say, for example, to figure out if the patient needs more Pitocin or to determine why dilation isn’t progressing), she’ll need to place a monitor inside the uterus. This internal contraction monitor gives a direct reading on how strong contractions are.

Similarly external fetal heart monitoring is less accurate than internal monitoring. If the provider wants heart information that comes directly from the baby (rather than through mom’s uterus), she’ll need to place a tiny electrode directly on the baby. Both of these procedures require AROM.
