Yet another possibility specific to back labor pain is the sterile water papule technique, which involves injecting sterile water just under the skin at four locations in the lower back. Although the injections can sting intensely as they are administered, pain relief is fairly complete and lasts for roughly an hour. The injections can be repeated, have no effect on your consciousness or ability to move about, and can be administered by a care provider in any setting.

For most women, once in second-stage labor, pain is no longer an issue. Yes, there is great distension of vaginal tissues, but pressure from the head causes a certain degree of numbness without limiting the sensitivity that facilitates orgasmic birth. As you feel this pressure build, focus on letting go and breathing your baby out. Barring an emergency need to get the baby out quickly, ignore advice from nurses or other attendants to push hard or steadily. Follow your body’s signals at all times, and keep that oxygen coming to the baby! Upright or forward-leaning positions are usually best at this point, but let your body decide: If the baby is coming down rapidly, you may choose a less dynamic position, such as side lying or on hands and knees.

I think the end of pushing, with the baby’s head coming out, is the most challenging and most surprising moment of a lifetime. At that point, it seems we are observers just as much as we are facilitators. In that moment, I am in touch with the most powerful and the most vulnerable parts of myself, and I deal with this by orchestrating some seriously loud noise!

Key points that could be extracted from my experience would be non-attachment and allowance of emotion—in my case, weeping and laughing. These deep-feeling states are part of tapping into our primal tools for releasing blockages, needed to express the baby through and out.
