6.2.1 Anatomy and Pathophysiology

Lateral elbow stability is maintained by an interaction of static and dynamic soft tissue restraints as well as the osseous congruency of the elbow. The lateral collateral ligament complex of the elbow consists of four structures—the radial collateral ligament (RCL), lateral ulnar collateral ligament (LUCL), accessory lateral collateral ligament, and annular ligament (Fig. 6.8) [2]. The LUCL and RCL originate at the lateral epicondyle and are indistinct at that location. From the lateral epicondyle, the RCL extends distally and blends with the annular ligament, which inserts along the anterior and posterior margins of the radial notch [28]. The LUCL extends distally to insert directly onto a tubercle of the supinator crest—the crista supinatoris [2].
