Cardiovascular drugs

Cardiovascular drugs have proven effective in extending the life span of a large proportion of the population. An up to date medical history is essential, where there is any doubt the patients medical practitioner should be contacted for guidance. The pulse and blood pressure should be checked for patients with a history of cardiovascular disease before each treatment commences.

Stable and managed cardiovascular disease is not a contraindication to delivery of dental therapy care. Epinephrine (adrenaline) vasoconstrictor should be limited to 0.04 mg in cardiac patients. Care must be taken to inject local anaesthetic slowly and use aspiration to prevent intravascular injection of the agent.

Common cardiovascular drugs include digitalis glycosides, angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, β-adrenergic blockers, antiarrhythmics. Always ensure GTN tablets or oral spray are brought to appointments and easily accessed during the appointment.

In some circumstances dental treatment should be delayed until medical approval to proceed has been obtained:

Acute or recent myocardial infarction.

Unstable or recent onset of angina pectoris.

Uncontrolled congestive cardiac failure.

Uncontrolled arrhythmias.

Significant uncontrolled hypertension.
