35.6 Prognosis

The prognosis of MMP is variable. Patients with mild disease may experience complete remission with therapy. Many patients, however, have a chronic course with episodes of exacerbations and remissions often leading to scarring. All the patients need a long-term follow-up in order to detect lesions occurring on other mucous membranes.

Functional prognosis is related to ocular and esophageal involvement. Ophthalmological lesions may get worse and eventually result in blindness. Esophageal stricture may result in severe dysphagia leading to significant weight loss.

Vital prognosis is engaged in case of scarring laryngeal involvement. Indeed laryngeal stenosis may result in asphyxiation and fatal outcome if no tracheostomy is performed.

Lastly a high incidence of mortality has been reported in patients with anti-laminin-5 MMP related to an increased risk for solid cancer and treatment with systemic immunosuppressives [24].
