Polio. Polio is a risk in parts of Africa and Asia. The four major countries in which polio is still considered endemic (constantly present in a specific population or area) are India, Pakistan, Afghanistan, and Nigeria (Niger and Egypt were taken off the endemic list in 2006, but some cases continue to strike there). In 2003–2004, an epidemic of polio spread through the entire sub-Saharan region of Africa. In 2007 and 2008, twelve countries in Asia and Africa that had been previously polio free experienced some cases imported from India or Nigeria. These small outbreaks were contained and resolved. I advise any unvaccinated person to get at least two shots of the polio vaccine before traveling to these countries. Three doses are needed for full immunity.

Meningococcal Disease. Countries in “the meningitis belt” in Africa (those countries that border the southern edge of the Sahara Desert) have a high rate of meningococcal disease between December and June. One dose is needed for protection.

Other Vaccine-Preventable Diseases on the Routine Vaccine Schedule. The following diseases don’t pose as much risk as the above, but prolonged interaction with the native populations (through nontourist travel) does pose a somewhat greater risk than you encounter in your daily life here at home:

HIB: This is still common in many countries. Consider vaccinating your infant if travel will involve mingling with many native kids. See note here for HIB dosing when it has been delayed.
