If your institution provides a prepared survey, you may want to modify it and make it shorter or longer to focus on getting feedback on a new activity or process that you are using.

Many survey tools are readily available. Two of the survey tools listed in the Top 100 Tools for 2015 by the Centre for Learning and Performance Technologies (http://c4lpt.co.uk) can be useful for early term feedback. One is SurveyMonkey (www.surveymonkey.com), an online survey tool that allows users to design surveys, collect responses, and analyze the responses of their created surveys. Another survey tool in the 2015 list is Quizlet (www.quizlet.com). This website provides learning tools for students, including flashcards, study and game modes. One other survey tool to consider is PollDaddy (https://polldaddy.com).
