Recommendations

The findings of the study need to be disseminated so that they can support the development of future health policy. A model for health facilitation has been provided in this study which can be adapted within other PCT areas. As an advanced nurse practitioner the project facilitator must now seek to influence the national agenda to ensure that strategies across the country target the best health outcomes for people with learning disabilities and ensure that resources are focused on these. The NHS Plan (Department of Health, 2000) promotes the role of nurses in taking a lead in influencing how local services are organised and run. The health facilitator is now in an ideal position to influence commissioners of PCTs and services.

Taking a health facilitation model forward

Now that an effective health facilitation model has been implemented across the city resources have been redirected to this area. This has led to the establishment of a citywide health facilitation team which is line managed by the project facilitator. See Figure 7.3.
