Iloperidone (Fanapt®)26

--Available in 1 mg, 2 mg, 4 mg, 6 mg, 8 mg, 10 mg and 12 mg tablets

--Target dosage is 12 to 24mg per day divided BID (titrate slowly to reduce orthostatic hypotension)

--Side effects: Dizziness, dry mouth, fatigue, nasal congestion, orthostatic hypotension, somnolence, tachycardia, and weight gain

--Not a first-line agent due to QTc prolongation

Asenapine (Saphris®)[183] – also approved for Bipolar

--Available in 5mg and 10mg sublingual tablets (black cherry flavored)

--For Schizophrenia: 5mg SL BID / For Bipolar Disorder: 10 mg SL BID

--Administration: Food and water should be avoided for 10 minutes after dissolving the tablet under the tongue. Do not swallow the tablet.

--Elimination of asenapine is primarily through direct glucuronidation by UGT1A4 and oxidative metabolism by cytochrome P450 isoenzymes (predominantly P450-1A2)

--Caution if given with QTc-prolonging medications or in cardiac-risk patients

--Serious allergic reactions have been reported to the FDA
