5.2.1.2 For a Retrospective Analysis

When calculating the base case ROI retrospectively , the appropriate values to use for the rates, reductions, and impacts are those that were observed over the course of the project. It will be likely that some or even all of the values observed during the activities will differ from what was anticipated (or hoped for), but for the base case it is usual to simply present the return that was actually realized.

The exception might be if there is not a comparison group or if data from that group are unavailable or unusable to viably help determine change or improvement, thereby making a value determination difficult or impossible. This could happen if (1) there were no data collected prior to the intervention and there was no control group or (2) something unattributable to the project changed during the intervention as to make a comparison to any pre-intervention or control group tenuous. Perhaps significant changes occurred to the patient population in some characteristic that are known to influence outcomes, for example. Or maybe external forces existed that were thought to introduce bias or uncertainty during your intervention. Changes in health policy, reimbursement, practice guidelines, and even the local and/or physical environment may reasonably affect the observed outcomes in a way that complicates or compromises the ability to make a reasonable comparison. In these circumstances, if comparison data are not available, it may be reasonable to use values from other sources like published literature or expert opinion, similar to when performing a prospective ROI analysis. Ideally, one would gather information from multiple resources and select the most reasonable, just like when amassing estimates for a prospective analysis.
