The Sacrum as a Wedge (Figs. 8.58 and 8.59)

The pelvic girdle must be stable to successfully transmit the body weight to the lower extremities. The ligamentous connections, the muscles, and the sacral wedge ensure this stability.

In the frontal view, the sacrum presents as a wedge that narrows at the inferior end and is wedged between the two hip bones.

In the transverse plane, the sacrum is wedged into the pelvic girdle. The strong posterior ligaments that overlay the SIJ and the ligamentous connections of the pubic symphysis stabilize the sacrum.

Pathology

Instability

If the ligaments lose stability at a particular point, the strength of the entire pelvic ring is impaired. During pregnancy, the release of hormones causes loosening of the ligaments of the pelvic girdle, making it more flexible to allow for the birth process. This loosening must normalize after birth; otherwise instability remains.

Disruption of the Pubic Symphysis (Fig. 8.60)

During the birth process, disruption of the symphysis can occur. X-ray and palpation can demonstrate dehiscence of the symphysis in which the pubic bones may drift up to 5 cm apart.

Therapy: application of a firm trochanteric belt.

Trauma

During a fall on both tuberosities, the ilia wedge the sacrum as they are pushed superiorly.

In pelvic fractures, the ability of the pelvis to bear weight may be lost, with a resultant loss of stability.
