Recent History Benefits from Ancient History

We must remember that people’s lives change, and, as James Morrison observes, “often the evolving symptoms of an illness will help you to evaluate it better. A diagnosis that seemed reasonable ten years ago may be untenable in the light of more recent symptoms.” 30 Consider the following examples:

A twenty-eight-year-old man with a diagnostic history of intermittent anxiety attacks marked by expansiveness and distractibility as a teenager was recently diagnosed with Major Depression, Single Episode. When the history was reviewed and the symptom pattern noted, the diagnosis was changed to Bipolar I Disorder, Most Recent Episode Depressed.

A shy, withdrawn sixteen-year-old girl was diagnosed with a Social Phobia. In college the struggles with low self-esteem, interpersonal avoidance, and hopelessness continued, and she was diagnosed as Major Depression, Single Episode. Shortly after graduation, she became delusional, accusing her fiancé of being repeatedly unfaithful. In the end she was diagnosed as Schizoaffective Disorder, Depressive Type.
