It is important to note that the emphasis in this chapter is on disability determination, because that is often the primary reason that adults seek evaluation in this author's practice. It is also a complex area for clinicians to understand. However, it is acknowledged that clients may seek evaluation because they or a referral source wish to understand the nature of their problems and what they can do about them. In such instances, the issue of disability documentation in the detail described here is less important.
