ߜ You must send the affidavit to the fraud department of each creditor, bank, or utility that provided the identity thief with unauthorized credit, or purchases and services.

Some companies have their own dispute forms; they will send them to you after they receive your affidavit. Still other companies do not accept the affidavit and will only accept their own dispute form and a police report, but you will not know until you send it them.

Figure 10-1 shows the sections of the report from the Federal Trade Commission Web site. Each of the sections is explained briefly in the next section.
