WHY? ANSWER
1 Why was the tea cold? Because the kettle did not heat the water properly.
2 Why did the kettle not work? Because the fuse burned.
3 Why did the fuse burn? Because they plugged it into the wrong socket.
4 Why did they plug it into the wrong socket? Because there is no three-phase power marking.
5 Why is there no marking? Because the maintenance team forgot to put the marking in the kitchen.

Remember, some causes may not be relevant. Itâs important to check all potential causes using the available facts and data. Where multiple causes are possible, then prioritisation of the causes helps ensure attention and activity are focused on the most relevant.
