Homeward Trust and its partners throughout the community establish crystal-clear targets. For example, the goal was set that, through assertive outreach and coordinated access to appropriate housing and supports, all rough sleepers would be engaged by 2018. In June 2020, there were 505 unsheltered individuals in contact with the city and on its by-name list.5 Agencies outside the homeless systems also play an important role; for example, corrections, health care, and child intervention are expected to report at least biennially on the number of people discharged into homelessness from public systems. Homeward Trust has committed to annual targets that aim to achieve zero discharge into homelessness by 2023, with the aspiration that 100 percent of those entering homeless-serving systems in 2022 will be connected to housing and supports within twenty-one days, effectively ending chronic and episodic homelessness.
