4.2.1 Case Presentation

A 74-year-old male, 0-IIc with scar, tub1, pT1a: M.

Fig. 4.4(a) White light image showed superficial depressed type lesion with ulceration (UL) scar. The lesion is located at the posterior wall of antrum. (b) After spraying with indigo carmine, a slightly elevated lesion with central depression was observed. The lesion is extended under full air conditions. (c) The lesion could be detected as a brownish area compared with background mucosa by narrow band imaging (NBI). (d) Yellow line is the margin of the lesion (demarcation line) by low power view of magnifying endoscopy with NBI (ME-NBI). (e) Loop pattern vessels and mesh pattern vessels are detected in the lesion by high power view of ME-NBI. The lesion is diagnosed as differentiated-type adenocarcinoma. In the lesion, biopsy specimens reveal well-differentiated-type adenocarcinoma. (f) Submucosal layer wedge-shaped deformity (white arrow) is detected by endoscopic ultrasound (EUS). There is no finding which suggests submucosal invasion in the lesion by EUS
