Proportion of Surgical Operations that are Inpatient Surgeries

The proportion of surgical operations that are inpatient surgeries is a proxy for the complexity of a hospital’s case mix. Generally speaking, inpatient surgeries are more serious than outpatient surgeries, and hospitals with a high proportion of inpatient operations tend to see patients in need of complex services.21 Therefore, a high level of inpatient surgery suggests a more intensive hospital environment. Conversely, hospitals that perform a limited number of inpatient operations see relatively minor cases.

Recent studies have examined the relationships among hospital surgical volume and patient mortality rates.22 Hospitals and surgeons with a high volume of specific surgeries have lower mortality rates than hospitals with low surgical volumes. As Stevens observes, “it is not surprising, then, that hospitals seeking to qualify for Medicare reimbursement for cardiac transplantation now have to meet specified conditions. These include preexisting open-heart surgery programs that perform at least 250 procedures a year, at least twelve heart transplants (on persons of any age) in each of the past two years, and specified survival rates (seventy-three percent after one year and sixty-five percent after two years), together with other criteria.”23 Table 10 presents inpatient surgeries as a proportion of all surgeries.
