Children and Adolescents

Generally, when children and adolescents are referred for ECT, they are quite ill. Mood disorders that are severe and start before adulthood are usually bipolar disorder or a mix of psychosis and mood symptoms, known as schizoaffective disorder. Psychotic illnesses that start before adulthood are also often severe. Antipsychotic medications usually provide some relief but may not induce a full remission. These patients get referred for ECT when they are acutely suicidal or at risk for assaultive behavior. Psychotic illnesses are chronic in nature, but ECT is usually effective in reducing the acute symptoms and reducing the suicidality.27 There are some small studies that suggest that ECT given at the start of the illness can reduce its severity over the course of the person’s lifetime.28
